Program Agreement | FERPA Release

I understand the Enhanced Admission Program between University of Health Sciences & Pharmacy in St. Louis (UHSP) and Webster University and wish to participate. I also understand that my personal and academic information, including academic transcripts, will be sent between Webster and UHSP in order to confirm eligibility in the Program, aid in communication between the institutions, and to facilitate my potential transfer to UHSP upon completion of my A.S. degree in Biology. I further understand that I must remain a student in good standing in order to maintain enhanced admission eligibility, and Webster has my permission to release information to UHSP pertaining to any student code of conduct violations. I understand that this release is valid for the duration of my participation in the Enhanced Admission Program between UHSP and Webster. I further understand that I may cancel or revoke this authorization at any time in writing, and it will remove me from the enhanced admission program.

Once you complete this form, an admissions specialist from UHSP will be in contact with you to further discuss your pathway to UHSP. For enhanced admission consideration, Webster University students should make a formal application to UHSP by December 1 of the year prior to their anticipated fall semester start at UHSP. UHSP will notify students by February 1st of the admission decision.
